AB-Abdomen Abdomen Sonography Examination Questions and Answers

The ultrasound image shown is a transverse endorectal (transrectal) ultrasound, commonly used to evaluate the prostate and adjacent structures. The two hypoechoic (dark) oval-shaped structures seen superior and posterior to the prostate are characteristic of the seminal vesicles.

The seminal vesicles are paired, elongated glands located superior and posterior to the base of the prostate and are best visualized in transverse planes on endorectal imaging. They appear as hypoechoic or anechoic structures with internal septations, depending on the degree of fluid content.

In contrast:

The urethra appears as a central echogenic linear structure within the prostate.

The prostate base is more inferior in the scan and is visualized just above the urethra.

The ejaculatory ducts are usually not as prominently visualized and are located medial to the seminal vesicles, entering the prostate near the verumontanum.

This image most clearly demonstrates the bilateral seminal vesicles.

The spleen is almost entirely covered by visceral peritoneum, except at the hilum where vessels, nerves, and lymphatics enter and exit. This area lacks peritoneal covering to allow vascular connection to the splenic artery and vein.

According to Moore’s Clinically Oriented Anatomy:

“The spleen is entirely covered by visceral peritoneum except at its hilum where the vascular structures enter.”

The ultrasound image provided shows a transverse view of the abdominal aorta, with a clearly measured aortic diameter of 5.71 cm. A normal adult abdominal aorta should measure less than 3.0 cm in anterior-posterior diameter. Any measurement exceeding this threshold is defined as an abdominal aortic aneurysm (AAA).

In this case, the dilation is well beyond the 3.0 cm threshold, confirming the presence of an aneurysm. The rounded, anechoic/heterogeneous central lumen surrounded by echogenic arterial wall layers further supports this diagnosis.

Comparison of answer choices:

A. Stenosis: Would show a narrowed lumen with turbulent, aliasing flow on Doppler, not a dilated aorta.

B. Dissection: Typically shows an echogenic intimal flap separating true and false lumens; no flap is visible here.

C. Aneurysm: Correct. The aorta’s transverse diameter (5.71 cm) confirms the presence of an aneurysm.

D. Occlusion: Would appear as a lack of flow with thrombus or echogenic content filling the lumen, not dilation.

Patients with liver disease often have coagulopathy due to impaired synthesis of clotting factors. This places them at greater risk for bleeding or hematoma formation after biopsy. While hypertension may increase bleeding risk slightly, liver disease presents a significantly higher risk due to impaired coagulation.

According to the Society of Interventional Radiology (SIR) guidelines:

“Liver dysfunction is a significant risk factor for post-biopsy hemorrhage due to associated coagulopathy.”

The image shows a transverse view of the left lobe of the liver with the portal triads clearly visible. The "parallel channel" or "double barrel" sign is observed—where dilated intrahepatic bile ducts run alongside the portal veins, creating a characteristic sonographic pattern of paired anechoic (black) tubular structures.

This sonographic feature is diagnostic for dilated intrahepatic bile ducts and is typically seen in obstructive jaundice or biliary obstruction from conditions such as:

Choledocholithiasis (stone in the common bile duct)

Stricture or mass compressing the bile ducts

Cholangiocarcinoma

The intrahepatic bile ducts normally are too small to visualize clearly unless dilated. Their dilation gives the liver a "too many tubes" appearance, where bile ducts become as prominent as the portal veins.

Comparison of answer choices:

A. Normal intrahepatic ducts are not usually seen this clearly or prominently on ultrasound.

B. Dilated intrahepatic — Correct. The parallel channel sign supports this diagnosis.

The ultrasound image demonstrates a longitudinal view of the right kidney. Within the renal pyramids, there are multiple echogenic foci, some showing posterior acoustic shadowing—findings characteristic of medullary nephrocalcinosis. This sonographic appearance is strongly associated with medullary sponge kidney (MSK).

Medullary sponge kidney is a congenital disorder of the renal tubules that results in ectatic (dilated) collecting ducts in the renal medulla. The dilated ducts frequently become calcified, leading to the "paintbrush" or "bouquet of flowers" appearance seen in the renal pyramids on ultrasound. These calcifications are most often bilateral and symmetrical, further aiding the diagnosis.

A. Fungal balls (mycetomas) typically appear as mobile, non-shadowing echogenic masses within the collecting system, often in immunocompromised patients.

B. Renal cell carcinoma usually presents as a solid mass with irregular borders and variable echogenicity—this image does not show a mass.

C. Acute pyelonephritis may show renal enlargement, decreased echogenicity, or loss of corticomedullary differentiation—but not calcification of the pyramids.

Type I choledochal cyst, characterized by fusiform dilatation of the common bile duct, is the most common form, accounting for 80-90% of cases. Other types are much less frequent.

“Type I fusiform dilatation of the extrahepatic bile duct is the most common type of choledochal cyst.”

In biliary obstruction (such as at the ampulla of Vater), dilation begins proximally and progresses peripherally. The intrahepatic peripheral bile ducts are the last to dilate because backpressure takes time to propagate. Early dilation is typically seen in the common bile duct and common hepatic duct.

According to Rumack’s Diagnostic Ultrasound:

“The intrahepatic peripheral bile ducts dilate last in the setting of progressive biliary obstruction.”

Conn syndrome (primary hyperaldosteronism) results from excessive aldosterone secretion, often due to an adrenal adenoma, leading to hypertension, hypokalemia, and metabolic alkalosis. Cushing disease involves cortisol, Addison disease involves adrenal insufficiency, and Waterhouse-Friderichsen is associated with adrenal hemorrhage.

According to Rumack’s Diagnostic Ultrasound:

“Conn syndrome is due to excessive secretion of aldosterone, often secondary to adrenal cortical adenoma.”

The clinical presentation—tender, swollen breast with erythema and fever—in a lactating female strongly suggests acute mastitis. The sonographic findings support this diagnosis. In the image, the breast parenchyma shows diffuse, hypoechoic, and heterogeneous echotexture with increased vascularity, which is consistent with inflammatory changes typical of mastitis.

Mastitis is a common complication during lactation, particularly in the first few weeks postpartum. It results from milk stasis and subsequent bacterial infection, commonly due to Staphylococcus aureus. Ultrasound features of mastitis include:

Ill-defined, hypoechoic, edematous areas in the breast parenchyma

Increased Doppler flow due to hyperemia

Skin thickening

Ductal dilatation may also be present

If left untreated, mastitis may progress to abscess formation, which would appear as a localized, complex fluid collection with peripheral hyperemia and internal debris. However, the image does not show a well-formed fluid collection consistent with abscess.

Option B (Ductal carcinoma): Inappropriate here due to the acute clinical scenario and patient age. Ductal carcinoma typically presents as a hypoechoic mass with irregular margins and posterior shadowing, not diffuse edema or inflammatory changes.

Option D (Galactocele): This benign milk-filled retention cyst typically appears anechoic or with fluid–fluid levels but lacks signs of inflammation and systemic symptoms such as fever.

Option A (Abscess): This could be a differential, but abscesses usually present with a well-defined anechoic or complex mass. The absence of a discrete collection and the diffuse appearance makes mastitis more likely.

The Doppler ultrasound image shows an elevated peak systolic velocity (PSV) of 637 cm/s, an elevated end-diastolic velocity (EDV) of 312 cm/s, and a low resistive index (RI) of 0.51 at the arterial anastomosis of a renal transplant. These findings are characteristic of significant renal artery stenosis (RAS) at the transplant vascular anastomosis.

Key sonographic features of renal artery stenosis:

Peak systolic velocity (PSV) > 250–300 cm/s at the stenotic segment (this case: 637 cm/s)

Post-stenotic turbulence with spectral broadening

Low resistive index (RI < 0.56 suggests downstream vasodilation)

Elevated acceleration time (AT > 0.07 sec), and reduced acceleration slope

Aliasing on color Doppler due to high velocity

In this image, the marked increase in velocity with spectral aliasing and low RI is diagnostic of transplant renal artery stenosis — the most common vascular complication post-transplant, typically occurring at the site of surgical anastomosis.

Differentiation from other options:

A. Iliac arteritis: A rare condition, not typically presenting with these Doppler changes.

C. Renal vein thrombosis: Would show reversed or absent diastolic flow, not elevated systolic velocities.

D. Arteriovenous malformation (AVM): Produces a high-velocity, low-resistance waveform but is associated with color bruit, aliasing, and pulsatile venous waveforms — not evident here.

Alpha-fetoprotein (AFP) is commonly elevated in patients with hepatocellular carcinoma (hepatoma), particularly in those with underlying cirrhosis or chronic hepatitis B/C. AFP is not typically elevated in adenomas, cholangiocarcinoma, or FNH.

According to Rumack’s Diagnostic Ultrasound:

“Serum AFP levels are elevated in 50–70% of patients with hepatocellular carcinoma.”

Exudative ascites is characterized by high protein content and cellular debris, typically resulting from infections (e.g., peritonitis, tuberculosis), malignancy, or inflammatory conditions. Transudative ascites is more commonly seen in conditions such as liver failure, renal failure, and hypoproteinemia due to changes in hydrostatic or oncotic pressure.

According to Light's Criteria:

“Infection and malignancy are common causes of exudative ascites, distinguished by high protein content and elevated LDH.”

The ultrasound image shows echogenic foci with dirty shadowing and reverberation artifacts within the gallbladder wall and lumen. These features are characteristic of emphysematous cholecystitis, a severe, life-threatening variant of acute cholecystitis caused by gas-forming organisms (e.g., Clostridium or E. coli) infecting the gallbladder wall.

Sonographic features of emphysematous cholecystitis:

Echogenic gas within the gallbladder wall or lumen

Reverberation or "dirty" shadowing artifacts

May show intramural gas bubbles or "ring-down" artifact

Often seen in diabetic or immunocompromised patients

No gallstones may be present ("acalculous cholecystitis")

Clinical context:

More common in elderly men and diabetics

Presents with right upper quadrant pain, fever, and leukocytosis

Surgical emergency due to risk of perforation and sepsis

Differentiation from other options:

A. Adenomyomatosis: Involves gallbladder wall thickening with “comet tail” artifacts due to Rokitansky-Aschoff sinuses, not intramural gas.

B. Porcelain gallbladder: Shows curvilinear calcification of the gallbladder wall — dense echogenic rim with posterior shadowing.

C. Gangrenous cholecystitis: May show wall irregularity, intraluminal membranes, and absence of Doppler flow but lacks intramural gas.

Postsurgical breast scarring may appear hypoechoic and irregular but typically shows no internal vascularity on Doppler imaging. In contrast, recurrent tumors generally exhibit increased internal vascular flow due to neovascularization. Therefore, absent Doppler signal helps suggest scar tissue rather than malignancy.

According to Stavros’ Breast Ultrasound:

“The absence of internal Doppler flow favors scar tissue, whereas recurrent malignancy typically demonstrates internal vascularity.”

An accessory spleen (splenule) is a congenital variant, commonly located near the splenic hilum, and has identical echogenicity to the native spleen. This finding is benign and often incidental.

According to Rumack’s Diagnostic Ultrasound:

“Accessory spleens are typically found near the splenic hilum and are isoechoic to the normal splenic parenchyma.”

Acute tubular necrosis (ATN) is the most common cause of early graft dysfunction following renal transplantation. It results from ischemia-reperfusion injury during the transplantation process. Ultrasound findings may be nonspecific but Doppler may show elevated resistive indices.

Ureterocele (A) is a congenital anomaly.

Transplant artery stenosis (C) is a late complication.

Transitional cell carcinoma (D) is rare and not typically an early complication.

The hallmark feature of focal nodular hyperplasia (FNH) is a central stellate scar seen on imaging. This fibrous scar may not always be seen on ultrasound but is a classic distinguishing feature from hepatic adenomas, which usually lack a central scar.

According to Rumack’s Diagnostic Ultrasound:

“Focal nodular hyperplasia often demonstrates a central stellate scar, which may be echogenic or isoechoic.”

The ultrasound image of the thyroid clearly shows posterior shadowing originating from the lateral edges of a rounded structure, which is indicative of edge shadow artifact. Edge shadowing occurs when an ultrasound beam passes tangentially to a rounded or curved structure, such as a cyst or blood vessel. The difference in sound wave refraction and beam divergence at the edges leads to decreased echo signals deep to the edges, creating linear hypoechoic bands — which is exactly what the arrows are pointing to in the image.

Edge shadow artifact is purely a result of beam physics and not a real anatomic or pathologic finding.

Key characteristics of edge shadowing:

Appears as a narrow, linear hypoechoic (dark) shadow extending deep to the edge of a curved interface (e.g., cyst, vessel, thyroid nodule)

Caused by refraction and beam deflection, leading to reduced beam intensity distal to the edges

Most commonly seen adjacent to cysts or fluid-filled structures

Differentiation from other options:

A. Focal enhancement: Appears as increased echogenicity distal to a fluid-filled structure due to lower attenuation of the sound beam through fluid (opposite of shadowing).

C. Speed error: A less common artifact that results in displacement of structures due to variation in assumed sound speed.

D. Comet tail: A reverberation artifact that appears as a series of closely spaced bright echoes, often associated with metallic objects or cholesterol crystals in adenomyomatosis.

Testicular metastases are rare and usually identified in older patients. The most frequent primary site of malignancies metastasizing to the testes is the prostate . Studies (Ulbright and Young, 2008; Mosharafa et al., 2003) indicate that prostatic adenocarcinoma accounts for the highest number of testicular metastases, with lung and gastrointestinal tract malignancies also contributing less frequently. These metastases can be unilateral or bilateral and are often discovered incidentally during surgical intervention for prostate cancer.

The metastatic route involves retrograde venous extension, arterial embolism, or lymphatic dissemination. Histologically, prostatic adenocarcinoma in the testis can be confirmed via immunohistochemical markers like prostate-specific antigen (PSA), supporting its prostatic origin.

The structure indicated by the arrow in the ultrasound image is the adrenal gland. On ultrasound, the adrenal gland in neonates and infants is relatively large and has a distinctive “Y” or “V” shape in the transverse view. It is located superior and slightly medial to the upper pole of the kidney.

In this image, the arrow is pointing to a hypoechoic, curved structure with a thin echogenic central stripe, representing the fetal adrenal gland. This echogenic stripe corresponds to the adrenal medulla, while the surrounding hypoechoic area represents the cortex.

Differential features:

A. Kidney: While the kidney is visualized posterior to the adrenal gland and shows a reniform shape with a central echogenic sinus and peripheral cortex, it is not the structure being directly pointed to by the arrow.

B. Bowel loop: Bowel has variable echogenicity with peristalsis and shadowing from air. It does not have the consistent morphology or location seen in the image.

C. Diaphragm: Appears as a thin, hyperechoic linear structure separating the thoracic cavity from the abdomen. It is seen more superiorly than the indicated structure and lacks the “Y” or “V” adrenal configuration.

Key Anatomical Landmarks:

The adrenal glands are located in the retroperitoneum, superior to the kidneys, and appear prominent on ultrasound in neonates.

In transverse view, the right adrenal gland is anterior to the crus of the diaphragm and posterior to the inferior vena cava (IVC).

In mild acute pancreatitis, the pancreas often appears diffusely enlarged and slightly hypoechoic due to edema and inflammation. However, in very early or mild cases, the pancreas may still appear normal. Heterogeneous echotexture may develop in more severe or necrotizing pancreatitis.

According to Rumack’s Diagnostic Ultrasound:

“In mild pancreatitis, the pancreas is commonly enlarged and hypoechoic due to inflammatory edema.”

The inferior vena cava (IVC) is formed by the confluence of the right and left common iliac veins at the level of approximately L5. The hepatic veins drain into the IVC superiorly but do not form it. The superior mesenteric and splenic veins join to form the portal vein, not the IVC.

According to Moore’s Clinically Oriented Anatomy:

“The IVC begins at the level of L5 by the union of the right and left common iliac veins.”

Hepatitis A is primarily transmitted via the fecal-oral route, often through contaminated food or water. Hepatitis B, C, and D are transmitted through blood and body fluids.

According to CDC and WHO guidelines:

“Hepatitis A virus (HAV) is transmitted primarily by the fecal-oral route via ingestion of contaminated food or water.”

The ultrasound image demonstrates diffuse increased echogenicity of the liver parenchyma with posterior beam attenuation (acoustic shadowing), findings that are consistent with hepatic steatosis (fatty liver disease). The liver appears brighter than normal, and the vascular markings, particularly of the portal veins, are obscured due to the increased parenchymal echogenicity.

Hepatic steatosis refers to the abnormal accumulation of fat within hepatocytes and is commonly associated with obesity, diabetes, alcohol use, and metabolic syndrome.

Classic sonographic features of hepatic steatosis include:

Diffuse hyperechogenicity (“bright liver”)

Poor visualization of intrahepatic vessels and diaphragm

Posterior acoustic attenuation

Increased hepatic echogenicity relative to the renal cortex

Differentiation from other options:

A. Acute hepatitis: Usually presents with normal or slightly decreased echogenicity, "starry sky" appearance due to prominent portal triads and periportal edema.

C. Medullary sponge kidney: A renal condition with echogenic medullary pyramids, not hepatic.

D. Acute medical renal disease: Affects the kidneys, often with bilateral renal enlargement and increased cortical echogenicity—again not related to liver imaging.

The sonographic image shows a complex fluid collection within the liver parenchyma, with internal echoes and possibly septations, consistent with an abscess. In the clinical context of post-procedural fever following a liver biopsy, a liver abscess is the most likely diagnosis.

A liver abscess appears on ultrasound as a hypoechoic or complex fluid collection that may contain internal debris, septations, or gas (which may produce reverberation artifacts). These features distinguish it from other post-procedural complications.

A cyst (Option A) typically appears as an anechoic, well-defined lesion with posterior acoustic enhancement and no internal debris—this does not match the image or clinical setting.

A biloma (Option B) is a bile collection that can appear similar to a cyst or fluid collection but typically occurs due to bile leak; however, fever and internal complexity on ultrasound more strongly suggest abscess.

A hematoma (Option D) may also appear complex but usually presents with pain and not fever unless secondarily infected. Over time, hematomas evolve in appearance but lack septations and gas unless superinfected.

Urinomas are collections of urine in the retroperitoneum that result from trauma, surgery, or obstruction causing urine leakage. Trauma is a frequent cause of urinoma formation due to disruption of the urinary tract.

According to Rumack’s Diagnostic Ultrasound:

“Urinomas may develop as a complication of trauma, surgery, or obstructive uropathy with urinary extravasation into the retroperitoneum.”

The tunica albuginea is a dense fibrous capsule surrounding the testis. Discontinuity of the tunica albuginea on ultrasound is diagnostic of testicular rupture — a urologic emergency that requires immediate surgical repair to preserve testicular function and viability. Early surgical intervention within 72 hours has a high success rate for testicular salvage (up to 90%).

Intratesticular hematoma (A) may be managed conservatively if the tunica albuginea is intact.

Heterogeneity of the parenchyma (C) indicates injury but not necessarily rupture.

Increased vascularity (D) may be seen with inflammation or reperfusion but does not mandate surgery unless rupture is present.

Changing the patient's position allows evaluation of lesion mobility. Blood clots and hematomas are often mobile, while true bladder wall masses remain fixed. This technique helps differentiate between solid masses and non-adherent debris.

According to Rumack’s Diagnostic Ultrasound:

“Changing patient position may distinguish between mobile blood clots and fixed bladder wall masses.”

The image shows a small hepatic lesion located very close to the anterior liver capsule, as indicated by the arrow. When imaging very superficial or near-field structures like subcapsular hepatic lesions, using a standoff pad is the most effective technique for optimizing visualization.

A standoff pad (also known as an acoustic stand-off or gel pad) helps increase the distance between the transducer and the superficial target. This improves the focus and beam shape for near-field imaging and minimizes reverberation and ring-down artifacts. It allows better evaluation of superficial lesions by positioning them within the focal zone of the transducer, which is usually set a few millimeters below the probe surface.

Differentiation from other options:

A. Decrease depth: While reducing depth can help center deeper lesions in the field of view, it does not address issues with near-field resolution.

B. Scan in upright position: This may help in gallbladder or fluid positioning but is not optimal for improving visualization of superficial liver lesions.

C. Move the transducer focus: Adjusting focus deeper into the image won't enhance resolution of very superficial structures unless a standoff is used to bring the lesion into the focal zone.

Hepatic adenomas are benign liver tumors strongly associated with long-term use of oral contraceptives and are most frequently found in women under 40. Hepatoblastoma is seen in children; hepatoma (HCC) is a malignant tumor typically found in cirrhotic livers. Cavernous hemangioma is unrelated to oral contraceptives.

According to Rumack’s Diagnostic Ultrasound:

“Hepatic adenomas occur predominantly in young women with a history of oral contraceptive use.”

In the presence of a nonocclusive thrombus, Doppler waveform above the thrombus typically shows dampened flow with loss of normal respiratory phasicity due to partial venous outflow obstruction. Complete absence of flow is typically seen with occlusive thrombus.

According to Zwiebel’s Introduction to Vascular Ultrasound:

“Partial obstruction produces dampened and continuous flow patterns superior to a nonocclusive thrombus.”

Histoplasmosis is a fungal infection that can lead to granulomatous disease. Chronic granulomatous infections may result in multiple splenic calcifications that appear as small echogenic foci with shadowing on ultrasound. Other infectious granulomas (e.g., tuberculosis) may present similarly.

According to Rumack's Diagnostic Ultrasound:

“Granulomatous infections such as histoplasmosis and tuberculosis may produce multiple splenic calcifications, often with shadowing.”

The ultrasound image provided shows the liver and diaphragm imaged in a coronal plane with characteristic rib shadows and costophrenic angles. The orientation of the image and the structures visualized suggest that the transducer is placed in the right mid-axillary line, with the sound beam directed coronally — this is a classic right coronal scanning approach.

Key features supporting this:

The liver appears superiorly in the image.

Multiple echogenic lines (representing the ribs) run obliquely, casting acoustic shadows.

The diaphragm and adjacent lung base are seen clearly, which is commonly imaged through the right intercostal spaces in a coronal plane.

Comparison of answer choices:

A. Anterior: Would show a more transverse view of the liver and not typically image the diaphragm and lung this way.

B. Posterior: Not used for upper abdominal scanning due to shadowing from the spine and posterior ribs.

C. Left coronal: Would show the spleen and left kidney — not the liver as seen here.

Prostate adenocarcinoma is by far the most common malignancy of the prostate gland, typically arising in the peripheral zone. Seminoma is a testicular tumor, sarcomas are rare in the prostate, and hamartomas are benign lesions.

According to Rumack’s Diagnostic Ultrasound:

“Adenocarcinoma accounts for the vast majority of prostate cancers and is typically located in the peripheral zone.”
